What Happens at Your All-on-4 Maintenance Appointment

Here’s what’s included in a typical visit:

1. Clinical Evaluation

We begin with a comprehensive assessment of your implants, prosthesis, and soft tissue health.
This includes:

  • Checking for redness, swelling, or signs of infection
  • Measuring pocket depths around implants
  • Examining the fit and stability of your prosthesis
  • Evaluating your bite (occlusion) for signs of imbalance

Digital photos or X-rays may be taken if needed to monitor bone levels or prosthetic wear.

2. Deep Cleaning Under the Prosthesis

Even with excellent home care, food particles and plaque can accumulate underneath your bridge. Our hygienist or doctor will use specialized ultrasonic and hand instruments to:

  • Clean under and around the prosthesis
  • Remove calculus (hardened tartar) that can’t be brushed away
  • Irrigate the area with antimicrobial solutions to reduce bacterial load

If the prosthesis is screw-retained and removable, it may be gently unscrewed during the appointment for a more thorough cleaning.

3. Implant Screw Tightness Check

Over time, chewing forces can cause micromovements in your bridge or even loosening of the screws. During the session, we check for:

  • Proper torque of abutment and prosthetic screws
  • Any signs of cracks, fractures, or material wear
  • Mobility or shifting that could affect implant health

If needed, the screws are retightened or replaced, and the prosthesis is reseated.

4. Occlusion (Bite) Adjustment

Small shifts in your bite can lead to overloading, jaw discomfort, or wear on the material. Dr. Romeo will evaluate and adjust your bite as needed to ensure:

  • Balanced contact between upper and lower teeth
  • Even pressure distribution across all implants
  • Long-term stability of the prosthesis and surrounding bone

5. Polishing & Final Rinse

Your prosthesis is polished with implant-safe pastes and brushes to restore shine and smoothness, reducing the chance of plaque accumulation.
We’ll also apply:

  • Chlorhexidine gel or rinse to reduce inflammation
  • Optional fluoride treatment for adjacent natural teeth (if any remain)

6. Home Care Review & Recommendations

We’ll walk you through any updates to your at-home care routine, including:

  • Brushing technique and frequency
  • Interdental cleaning tools (Waterpik, super floss, brushes)
  • Nightguard use (if you grind your teeth)
  • Recommended oral care products

You’ll leave with a customized care plan and product list tailored to your prosthesis material and lifestyle.

How Often Should I Have Professional Maintenance?

Most All-on-4 patients benefit from professional maintenance every 6 months. However, if you have risk factors—such as diabetes, smoking, or a history of periodontal disease—you may need to be seen every 3–4 months.
