As a first step, prospective All-on-4 patients undergo a comprehensive medical and dental evaluation to assess overall health and bone quantity and analyze dento-facial esthetics and occlusion.
Advanced radiographic imaging (CBCT), intraoral scans of current dentition and photographs are collected to ensure optimal treatment planning.
In one surgical procedure, two implants are positioned vertically in the anterior region of the jaw, while two implants are tilted posteriorly to avoid anatomical structures and maximize implant-to-bone contact. In some cases, additional implants can be insetted to improve the stability of the fixed prostheses. Once the implants are placed, one of our highly trained dental technicians will digitally design and manufacture a customized set of temporary fixed teeth. These new teeth will be delivered immediately after the surgery or within 24 hours, allowing patients to enjoy restored oral function and esthetics.
Once the implants have being integrated in the jawbone (usually after 3-6 months), permanent teeth made with composite material or zirconia will be fabricated.
Our dental hygienists will work with patients to create a customize recall program based on individual risk factors and characteristics. This will allow patients to maintain the beauty of their smile and to prevent complications.

All-on-4 is an ideal solution for patients who have lost all their teeth or have a failing dentition that cannot be used to hold conventional bridges or partial dentures. With one minimally invasive surgical procedure, it is possible to insert the All-on-4 implants and fixed prosthesis the same day, avoiding extensive bone grafting and the use of dentures. Bone grafting often increases treatment time, and can have a lower success rate in medically compromised patients or individuals taking specific medications. Dr. Davide Romeo has been treating patients with the All-on-4 protocol since 2004, conducting scientific studies and training clinicians on the surgical and restorative aspects of the procedure.
What are the steps of the All-on-4 treatment?
The first step to assess whether you may be a candidate for the All-on-4 protocol is to review your medical and dental history, followed by a comprehensive clinical examination that takes into consideration the esthetic parameters of your face and smile, the quantity of residual bone and gingiva, and the overall health of your temporomandibular joints. Implant position will be determined using a dedicated software that analyzes the amount of existing bone and the position of your final teeth. Surgery is performed under intravenous sedation and with the assistance of a qualified anesthesiologist. After the implants have been placed, temporary fixed teeth will be delivered immediately or within 24 hours, depending on the individual case. Once the biological integration of your implants has occurred, usually between four to six months, the restorative phase for the fabrication of your definitive teeth can begin.

How long will the All-on-4 treatment last?
The All-on-4 treatment can theoretically last a lifetime, as long as you take care of the implants and the prosthesis properly. This includes maintaining a daily dental hygiene routine, controlling risk factors (smoking and bruxism, in particular) and setting up periodic professional cleanings during which we will monitor the condition of your implants and teeth. Mechanical and biological complications can always occur, but prevention has been demonstrated to be the most effective method to achieving long term success.
