All-on-4: Questions and Answers

Considering full-arch dental implants can feel like a big step — and it’s completely natural to have questions. At Milano Dental Studio, we believe that informed patients make confident decisions, which is why we’ve created this FAQ section to help you better understand the All-on-4® treatment concept.
Below, we’ve answered some of the most common questions we hear from patients. If you don’t see your question here, don’t worry — we’re always happy to talk with you directly. Just reach out to schedule a consultation.

All-on-4® is an advanced dental implant technique used to replace an entire arch of missing or failing teeth with a fixed, full-arch prosthesis. It uses just four strategically placed implants to support a full set of upper or lower teeth — often completed in a single day.

Most patients who are missing multiple teeth, have failing dental work, advanced mobile teeth or are struggling with loose dentures are good candidates. Even if you’ve been told you don’t have enough bone for traditional implants, All-on-4® may still be an option — thanks to the use of tilted implants that avoid areas of reduced bone quantity.

The procedure is typically performed under local anesthesia with IV sedation, so you’ll be comfortable throughout. Most patients report minimal discomfort afterward, similar to having teeth extracted. Mild swelling or soreness may occur for a few days, but is usually manageable with over-the-counter pain medication.

IV sedation offers a deeper level of relaxation and comfort during the All-on-4® procedure. While you remain conscious and able to respond to instructions, you’ll feel extremely calm — and many patients don’t remember the procedure at all afterward. The main advantage is that it significantly reduces anxiety, making it an ideal option for patients who feel nervous about dental surgery. It also allows for a more comfortable experience during longer or more complex procedures, enabling the dental team to work efficiently while you stay relaxed.

Yes — that’s one of the biggest advantages of All-on-4®. In most cases, a temporary fixed prosthesis is placed on the same day as your implant surgery, so you leave with a full smile. After a few months of healing, your final, permanent prosthesis will be placed.

Initial healing takes a few days to a week, and most patients return to normal activities within a few days. Complete integration of the dental implants into the jawbone typically takes 3–4 months, during which your temporary prosthesis allows you to smile, speak and eat comfortably.

Daily hygiene is essential. You’ll need to:

  • Brush twice a day with a soft toothbrush and non-abrasive toothpaste
  • Use an interdental brush or water flosser to clean under the prosthesis
  • Rinse with an antibacterial mouthwash as recommended
  • Regular professional cleanings and check-ups are also critical to long-term success

Yes — the prosthesis is designed to closely mimic natural teeth in both appearance and function. It is custom-made to suit your facial features, bite, and smile line, giving you a result that looks and feels natural.

With proper care, the implants can last 20 years or more, and often a lifetime. The prosthetic teeth may need maintenance or replacement after 10–15 years, depending on the material and wear.

As with any surgery, there are some risks, such as infection, implant failure, or gum irritation. However, complications are rare when the procedure is performed by an experienced team. Regular follow-ups and excellent oral hygiene help minimize risks.

The cost varies depending on your individual needs, including the number of arches treated and whether additional procedures (like extractions) are needed. At Milano Dental Studio, we offer personalized treatment plans and financing options to make care more accessible.
